The status and influencing factors of kinesiophobia in patients with diabetic peripheral neuropathic pain

Objective To understand the status and influencing factors of kinesiophobia in patients with diabetic peripheral neuropathic pain(DPNP). Methods From February to June 2023,192 patients with DPNP in a tertiary general hospital in Nanchang City,Jiangxi Province were selected by convenience sampling method. The patients were investigated by the demographic information questionnaire,Tampa Scale for Kinesiophobia,Numerical Rating Scale,Medical Coping Modes Questionnaire,Diabetes Distress Scale,and Summary of Diabetes Self-care Activities.Multiple linear regression were used to analyze the influencing factors of kinesiophobia in patients with DPNP. Results The score of kinesiophobia in patients with DPNP was(36.68±3.88),and the incidence of kinesiophobia was 38.5%. Multiple linear regression analysis showed that marital status,pain level,coping style,diabetic distress and self-management behavior were influential factors for kinesiophobia in patients with DPNP(all P<0.05). Conclusion Patients with DPNP have a moderate level of kinesiophobia. Nursing staff should pay attention to the early assessment of patients’ kinesiophobia,promptly identify and take effective measures to reduce the level of patients’ kinesiophobia,increase their motivation to exercise,and improve their prognosis and quality of life.

Meta-synthesis of qualitative research on the experience of self-management behavior changes for cancer patients

Objective To systematically evaluate the self-management behavior transformation experience of cancer patients,providing a basis for the subsequent development of targeted self-management intervention measures. Methods Qualitative studies related to self-management behavior transformation experience of cancer patients from Cochrane Library,CINAHL,Embase,PubMed,Web of science,PsycINFO,and China National Knowledge Infras-tructure,VIP Database,Wanfang Database,and China Biomedical Literature Database were searched by computer,with a search deadline of July 2023. The literature quality evaluation was conducted using the qualitative research quality evaluation standards of the Evidence Based Health Care Center at the Joanna Briggs Institute in Australia,and the results were integrated using a pooled integration method. Results A total of 12 articles were included,and 50 research results were extracted and summarized into 14 new categories. 4 integrated results were synthesized,including the forms of self-management behavior transformation,the characteristics of self-management behavior transformation,the driving factors of self-management behavior transformation,and the obstacles to self-management behavior transformation. Conclusion Medical staff should pay attention to the real experience of cancer patients’ self-management behavior transformation. Targeted ability cultivation and psychological intervention can be carried out based on the characteristics and influencing factors of cancer patients’ self-management behavior transformation,promoting their smooth transformation of self-management behavior.

Current situation and strategies for improving Traditional Chinese Medicine health education among hemodialysis patients in 7 tertiary TCM hospitals

Objective To understand the proficiency level and sources of Traditional Chinese Medicine(TCM) health education among hemodialysis patients,in order to provide the basis for promoting the application of TCM nursing in hemodialysis patients. Methods Utilizing a convenience sampling approach,we conducted a survey from April to May 2023 involving hemodialysis patients from 7 tertiary TCM hospitals across Beijing,Hebei Province,Sichuan Province,and other regions. Custom-made questionnaires were utilized,gathering general information,responses concerning the level of mastery on TCM health education,and their primary sources of such knowledge. Results A total of 413 questionnaires were disseminated,and 392 proved valid,yielding a valid response rate of 94.92%. The score of patient’s knowledge on TCM health education averaged(8.25±3.73),and an average score rate of 48.53% was obtained. In the patients undergoing hemodialysis,a relatively larger number of people possess knowledge of TCM health knowledge about relieving symptoms(50.8%~55.9%) and traditional Chinese daily life routines(56.4%~90.1%). However,fewer people are aware of how to use the five-tone therapy method for emotional inter-vention (40.8%) and principle of taking restorative Traditional Chinese Medicine (24.5%~36.7%),the dietary prin-ciples of “nurturing yang in spring and summer, and nurturing yin in autumn and winter” (14.3%),and theories of TCM kidney functions (9.9%). Approximately 23.0% of the patients sourced their TCM health education knowledge from new media platforms. Compared with new media method alone,patients who utilized only traditional educational platforms(P=0.020),and those who combined both new media and traditional methods(P=0.018) demonstrated higher proficiency in TCM health education. Conclusion Hemodialysis caregivers are urged to develop a TCM health education framework that emphasizes emotional well-being and traditional wellness concepts,thereby fostering patient-centric TCM health ideologies. Hemodialysis education personnel are encouraged to leverage new media whilst ensuring education quality and effective outcomes.

Evidence-based practice for the prevention and care of ocular complications in ICU patients

Objective To develop an evidence-based practice program for ICU eye care and evaluate its application effect using Stetler model of research utilization. Methods The systematic search was carried out in domestic and foreign guidelines,systematic reviews,evidence summaries,expert consensuses on eye care for ICU patients. The quality of the included literature was evaluated and the evidence was summarized. The evidence based on expert advice,clinical context and stakeholder opinions was screened. According to Stetler model of research utilization,the evidence translation/application was formulated from 3 aspects,including method,level and type of evidence application,and the evidence translation was implemented using a stepwise trial design. Five ICUs in a tertiary hospital in Zhejiang Province were selected,and the order of enrollment of each ICU was determined by computer-generated random numbers from April to september,2022. In the first month,all ICUs were in pre-evidence-based practice period,and in the second month,an ICU was trained and entered the evidence-based practice period. Until the 6th month,all ICUs entered the evidence-based practice period. Then the rate of ocular symptoms and implementation rate of eye care measures were compared before and after EBP. Results A total of 1 540 patients were included. With regard to the implementation rate of eye care measures,the rate of lubricant use and suspected ocular infection management in patients at high risk for ocular complications in evidence-based practice period was significantly higher compared with pre-evidence-based practice period(P<0.001),and no statistically significant difference was found in the rate of eyelid closure. The incidence of conjunctival exposure,subconjunctival hemorrhage,conjunctival congestion,and ocular discharge in non-neurology critical care patients decreased compared with pre-evidence-based practice period(P<0.05),and the incidence of conjunctival edema and conjunctival congestion in NICU patients decreased compared with pre-evidence-based practice period as the intervention continued(P<0.001). Conclusion Evidence-based practice in eye care for ICU patients using Stetler model of research utilization can standardize the practice of eye care for ICU patients and improve the quality of care.

Objective To evaluate the systematic review of the cardiac rehabilitation adherence assessment tools,so as to provide references for the selection of appropriate research tools. Methods We searched the American Association of Cardiovascular and Pulmonary Rehabilitation website,the National Institute for Health and Care Excellence website,PubMed,CINAHL,PsycINFO,Embase,Web of Science,CNKI,Wanfang database,VIP database and Chinese Biomedical Literature database for published studies on cardiac rehabilitation adherence assessment tools for patients with cardiovascular disease,and the search time limit is from the establishment of the databases to December 3,2022. 2 researchers independently screened the literature,extracted data,and evaluated the methodological quality and the psychometric properties of the included studies according to the consensus-based standards for the selection of health measurement instruments,and the modified grading of recommendations assessment,development of evaluation was used to classify the grade of evidence and recommendations of the tools. Results A total of 12 studies were included,involving 7 cardiac rehabilitation adherence assessment tools. None of the 7 assessment tools reported cross-cultural validity or measurement invariance,measurement error or respon-siveness;the Cardiac Rehabilitation Adherence Tool is Category C and not recommended due to high quality evidence of inadequate internal consistency. The remaining 6 assessment tools are of moderate or low quality of evidence due to uncertainty of content validity or inadequate internal consistency,and all are category B and recommended. Conclusion Compared with the other 6 assessment tools,the Cardiac Rehabilitation Barriers Scale was evaluated more comprehensively for its psychometric properties with good reliability and validity,and it can be provisionally recommended for use. However,more high-quality evidence is still needed to further explore assessment tools for cardiac rehabilitation adherence in patients with cardiovascular disease.

A multicenter survey on nurses’ knowledge and practice of children’s sleep management in pediatric ICU

Objective To investigate the current status of nurses’ knowledge,attitude and practice regarding sleep management of critically ill children in pediatric ICU,and to analyze its impact factors. Methods A self-designed questionnaire on general information and a questionnaire on knowledge and practical behaviors of pediatric ICU nurses on child’s sleep management were used. In March 2023,902 pediatric ICU nurses from 24 hospitals in China were surveyed using a convenient sampling method,and the impact factors were analyzed using multiple stepwise linear regression. Results 893 valid questionnaires were collected and the recovery rate of valid questionnaires was 99.00%. Nurses in pediatric ICU scored(33.71±7.76) in knowledge dimension,(37.38±4.86) in attitude dimension and(80.60±16.78) in practice dimension,with a total score of(151.78±24.27). The scores of knowledge and attitude,knowledge and practice,attitude and practice are all positively correlated (r=0.393,P<0.001;r=0.495,P<0.001;r=0.320,P<0.001). The results of multiple stepwise linear regression analysis showed that gender,region,whether they had received sleep management training were the influencing factors of pediatric ICU nurses’ total score of knowledge,attitude and practice towards children’s sleep management(P<0.05). Conclusion Nurses in pediatric ICU are positive about sleep management for critically ill children,but their knowledge and practice levels need to improve. Nursing managers should strengthen the theoretical knowledge and practical behavioral training of pediatric ICU nurses on child sleep management,develop scientific sleep management plans,and guide nurses to make reasonable evaluation and interventions to improve children’s sleep quality.

Best evidence summary for skin care in patients with hematopoietic stem cell transplantation complicated by graft-versus-host disease

Objective To search,evaluate and integrate relevant evidence of skin care for patients with hematopoietic stem cell transplantation complicated by graft-versus-host disease(GVHD). Methods Based on the “6S” pyramid model,the evidence was systematically searched in computer decision support systems,websites of guidelines,relevant professional associations and organizations and comprehensive databases,and evidence of skin care for patients with hematopoietic stem cell transplantation complicated by GVHD were screened from the establishment of databases to May 2022,including guidelines,clinical decisions,evidence summaries,systematic reviews,and expert consensuses. The quality evaluation,evidence extraction and integration of the evidence were conducted by 6 research members. Results A total of 15 pieces of literature were included,including 2 clinical decisions,6 guidelines,5 expert consensuses and 2 systematic reviews. 13 pieces of best evidence were summarized from 4 aspects,including multidisciplinary cooperation,skin assessment,nursing interventions and health education. Conclusion This study systematically integrated the relevant evidence of skin care for patients with hematopoietic stem cell transplantation complicated by GVHD,and provided evidence-based references for the development of relevant guidelines and clinical practice standards. When applying the evidence,nurses are recommended to com-prehensively consider the actual clinical situations and patients’ wishes,and select evidences prudently.

At present,the level of psychological resilience of domestic nurses is generally low. The construction of a scientific and effective resilience intervention program for clinical nurses is an urgent need to promote their physical and mental health and the stable development of the nursing group in China. This paper reviews the current research progress of resilience intervention for clinical nurses at home and abroad,summarizes the intervention form,intervention content and intervention outcomes,analyzes the shortcomings of current research,and puts forward the outlook for future intervention research. It aims to provide guidance for proposing scientific and reasonable intervention programs in the future.

Objective To analyze the potential categories and influencing factors of care dependency trajectory among first stroke patients. Methods From October 2021 to August 2022,278 first stroke patients in the Department of Neurology of a tertiary hospital in Zhengzhou were selected as the study subjects by the convenience sampling method. The Care Dependency Scale was used to investigate the patient’s care dependency level on 3 days after admission,at discharge and 1,3,6 months after discharge,and the latent growth mixture model,univariate analysis and multiple logistic regression were used for data processing. Results The developmental trajectory of 278 first stroke patients’ care dependency was divided into 4 latent subgroups,including low dependence-high recovery(42.1%),medium dependence-medium recovery(24.8%),medium dependence-high decline(13.3%) and high dependence-low recovery(19.8%). The influencing factors included age,Charlson complication index value,stroke type,stroke severity,anxiety,depression and nutrition score(P<0.05). Conclusion There are heterogeneous trajectories of care dependency in first stroke patients. Medical staff can make individualized interventions according to the influencing factors to reduce the degree of care dependence.

The reliability and validity test of the Chinese version of the Tyndall-Bailey Falls Risk Assessment Tool for ICU

Objective To sinicize Tyndall-Bailey Falls Risk Assessment Tool For ICU (ICU-FRAT) and test its reliability and validity. Methods Based on Brislin model,the original scale was translated,back-translated,checked and translated,cultural adjusted and pretest to form the Chinese version of ICU-FRAT. A total of 200 ICU patients from 3 hospitals in Nanjing from March to May 2022 were investigated by convenience sampling to evaluate the reliability and validity of the scale. Results The Chinese version of ICU-FRAT has a total of 10 items,with item level content validity index of 0.83 to 1.00,overall consistency level content validity index of 0.90,and average level content validity index of 0.98. After exploratory factor analysis,3 common factors were extracted,and the cumulative variance contribution rate was 79.32%. Confirmatory factor analysis suggested that each test quantity of scale model fit was within an acceptable range. The Cronbach’s α coefficient of the total scale was 0.857;the Cronbach’s α coefficient of each item ranged from 0.830 to 0.867;the half reliability was 0.838. Conclusion The Chinese version of the ICU-FRAT has good reliability and validity,and it can be used to assess the risk of falls in ICU patients.

Study on the application of a phased oral feeding promotion program in preterm infants with gestational age less than 32 weeks

Objective To investigate the effect of a phased oral feeding promotion program in preterm infants with gestational age less than 32 weeks. Methods 128 preterm infants less than 32 gestational weeks admitted to the neonatal intensive care unit(NICU) of a tertiary Children’s Hospital in Shanghai from January to December 2020 were consecutively enrolled as the subjects of this study. They were randomly divided into an experimental group and a control group by block method. The modified phased oral feeding promotion program was applied in the experimental group. The control group received the traditional feeding method. The days of life,corrected gestational age and weight of 2 groups at attaining full oral feeding,at the beginning of oral feeding and the first oral intake of 50% of the meal were compared. The hospital stays and the occurrence of feeding related complications were also compared. Results There were 5 cases lost,and finally 61 cases were included in the experimental group and 62 cases were included in the control group. The corrected gestational age at attaining full oral feeding in the experimental group was lower than that in the control group(P<0.05). The days of life,corrected gestational age and body weight of the experimental group were significantly lower than those of the control group at the beginning of oral feeding and the first oral intake of 50% of this meal(P<0.05). There was no significant difference between 2 groups in hospital stay(P>0.05). No feeding related complications occurred in 2 groups. Conclusion For premature infants with gestational age less than 32 weeks,the phased oral feeding promotion program can achieve full oral feeding at an earlier time.

Loneliness is common in patients undergoing hematopoietic stem cell transplantation (HSCT) during protective isolation,which could affect their health outcomes. This article reviews the latest studies on loneliness,including concept,theory,influencing factors,assessment tools,and interventions. Currently,there are few studies on loneliness in HSCT patients in protective isolation in China and other countries. We propose to conduct some research to investigate the current status of loneliness,the influencing factors,and the relationship with patients’ mental health problems in the future,which could provide a theoretical basis for developing interventions to reduce the loneliness of the HSCT patients in protective isolation.

Construction on a decision-making behavior model of blood glucose management among pregnant women with gestational diabetes mellitus

Objective To construct the decision-making behavior model of blood glucose management among pregnant women with gestational diabetes mellitus(GDM) based on protection motivation theory. Methods A total of 1 261 pregnant women with GDM from 6 hospitals in Shanghai and Jiangsu province were selected by convenient sampling and investigated through the cross-sectional survey. The Self-Developed Blood Glucose Management Decision-Making Behavior Questionnaire,Protection Motivation Questionnaire,Knowledge Questionnaire and General Information Questionnaire were used to collect data. Multi-linear regression analysis was used to identify the influencing factors of decision-making behavior. Structural equation model was used to establish and revise the decision-making behavior model of blood glucose management. Results 1 181 pregnant women with GDM were included in this study. Vulnerability,severity,response efficacy,self-efficacy and response costs were influencing factors of blood glucose management decision-making behavior(P<0.001). Perceived severity(β=0.204),vulnerability (β=0.196) and self-efficacy(β=0.336) had a direct positive influence on decision-making behavior. Perceived vulnerability had an indirect positive influence on decision-making behavior mediated by self-efficacy(β=0.136). Response costs(β=-0.186) had a direct negative influence on decision-making behavior. Knowledge had an indirect influence on decision-making behavior mediated by response costs(β=0.016). All the indicators for fit test were acceptable or optimal,and the path coefficients of model reached significant level(P<0.05). Conclusion The decision-making behavior of blood glucose management among pregnant women with GDM could be improved through increasing perceived severity,vulnerability and confidence,providing more knowledge and decreasing barriers related to blood glucose management.

Development of a risk prediction model for intraoperative abnormal blood glucose in elderly patients without diabetes

To analysis the risk factors of intraoperative blood glucose abnormalities(dysglycemia) in elderly patients without diabetes,and to develop a nomogram risk prediction model. Methods Convenience sampling was conducted in elderly patients aged ≥ 65 years without diabetes who received surgical treatment. Univariate analysis and binary logistic regression analysis were used to determine the risk factors of intraoperative blood glucose abnormalities,establish a risk prediction model and draw a nomogram. Receiver Operating Characteristic(ROC) and Hosmer-Lemeshow tests were used to verify the predictive effect of the model,and Bootstrap method was used for internal validation. Results The logistic regression analysis showed that type of anesthesia,duration of surgery,baseline operating room blood glucose level,and age were independent predictors of intraoperative dysglycemia. The predictive formula for intraoperative dysglycemia was established as follows:Logit P=-12.810+0.066 × age +1.966 × baseline operating room blood glucose level +0.008 × duration of surgery -2.778 × type of anesthesia. The area under the ROC curve was 0.815,and the optimal critical value boundary was 0.765. The sensitivity and specificity were 83.00% and 67.00%,respectively. The result of Hosmer-Lemeshow test was χ2=5.557(P=0.697). The prediction curve fit well with the ideal curve,suggesting that the model has good predictive ability. External validation showed that the sensitivity of the model was 77.01%,the specificity was 75.36%,and the overall accuracy was 76.56%. Conclusion The prediction model constructed in this study has a good effect,which can provide a reference for clinical evaluation of the risk of abnormal blood glucose in elderly patients without diabetes.

Objective To investigate the status of recurrence risk perception in first-episode ischemic stroke patients,and to analyze the influencing factors. Methods Based on the theory of triadic influence,a convenience sampling method was used to select 236 patients with first-episode ischemic stroke who were reexamined in neurology outpatient clinics of 2 tertiary general hospitals in Xi’an and Xianyang,Shaanxi Province from December 2021 to April 2022 as the research subjects. General Information Questionnaire,Recurrence Risk Perception Scale for Patients with Stroke,Health Literacy Scale for Stroke Patients,Patient Communication Pattern Scale and Measure of Stroke Environment were used to investigate the patients. Results The total score of Recurrence Risk Perception Scale for Patients with Stroke in 236 patients with first-episode ischemic stroke was(31.89±7.66) points. Multiple linear regression analysis showed that educational level,time of diagnosis,number of risk factors for ischemic stroke recurrence,family history of ischemic stroke,health literacy,doctor(nurse)-patient communication pattern,and environmental experience were the influencing factors of recurrence risk perception in first-episode ischemic stroke patients(P<0.05). Conclusion The ability to perceive recurrence risk in first-episode ischemic stroke patients is at a moderately low level. Individualized interventions can be carried out for patients with different characteristics to promote the patients’ ability to perceive recurrence risk and the formation of preventive behaviors.

Objective To develop a multi-module rehabilitation nursing model,and apply it in intelligent follow-up of stroke patients,and evaluate the effect on symptom experience and quality of life of stroke patients. Methods From January 1,2020 to October 31,2021,a total of 316 stroke patients in the Department of Neurology of a tertiary A hospital in Xinjiang Uygur Autonomous Region were selected by convenience sampling. According to the order of admission time,160 patients admitted from January 1,2021 to October 31,2021 were selected as the experimental group. A total of 156 patients admitted from January 1 to October 31,2020 were selected as the control group. The historical control method was used to verify the effect of multi-module rehabilitation nursing model on symptom experience and quality of life of stroke patients during intelligent follow-up. Results The symptom experience of the experimental group was improved compared with that before intervention;the quality of life and self-management behavior were improved;compared with the control group,the differences were statistically significant(P<0.05). Conclusion The application of multi-module rehabilitation nursing model in intelligent follow-up of stroke patients can reduce or alleviate the symptom experience of stroke patients,and effectively improve their quality of life and self-management behavior.

Construction of a preoperative risk prediction nomogram model for hypothermia during off-pump coronary artery bypass graftin

Objective To investigate the preoperative risk factors of intraoperative hypothermia in patients undergoing off-pump coronary artery bypass grafting(OPCABG),and to construct a nomogram prediction model. Methods A total of 202 patients undergoing OPCABG in a tertiary hospital in Beijing from January 2019 to June 2021 were selected by convenience sampling. The patients were divided into a hypothermia group(n=103) and a non-hypothermia group(n=99) according to whether they had hypothermia during operation. Relevant preoperative data were collected through the hospital electronic medical record system. Logistic regression was used to analyze risk factors. R software was used to draw a nomogram prediction model for intraoperative hypothermia in patients undergoing OPCABG. Hosmer-Lemeshow test was used to judge the goodness of fit of the model. The area under ROC curve was used to detect the predictive effect of the model. Results The incidence of intraoperative hypothermia in patients undergoing OPCABG was 50.99%. 5 factors were included in the intraoperative hypothermia nomogram prediction model,namely age,body surface area,ejection fraction,operating room temperature and body temperature at the beginning of disinfection. The Hosmer-Lemeshow test results of the model showed that P=0.471; the area under the ROC curve was 0.840(0.787~0.894);the optimal critical value was 0.344;the sensitivity was 0.903;the specificity was 0.616;the accuracy of external validation was 78.22%. They suggested that the model had good fitting effect and high predictive value. Conclusion The incidence of intraoperative hypothermia in patients undergoing OPCABG is high. The nomogram model constructed based on preoperative risk factors in this study can predict the risk of intraoperative hypothermia in patients undergoing OPCABG early and efficiently. It provides a scientific basis for clinical nursing staff to evaluate and effectively prevent intraoperative hypothermia.

To summarize the nursing experience of 3 patients with end-stage heart failure and critical complications after implantation of left ventricular assist device. The main points of care are as follows. A management team was established to formulate the best postoperative management plan,closely monitor circulatory function,and guard against the occurrence of right heart failure. The systematic anticoagulant management was implemented to monitor coagulation function;the continuous ECG monitoring and attention to arrhythmia were conducted;dynamic management of blood sodium level and timely treatment of hypernatremia were carried out; psychological factors were actively adjusted to reduce the risk of delirium. After careful treatment and nursing care,all the 3 cases were transferred to the general ward successfully,and were discharged at 56,29,29 days after surgery,respectively.

Objective To describe and analyze the experience of parents of non-suicidal self-injury(NSSI) adolescents in caregiving and coping,and to provide a basis for developing family support programs. Methods It was a descriptive phenomenological study. Purposive sampling was used to select 19 parents of NSSI adolescents who sought medical treatment at a tertiary hospital in Guangzhou from August to October 2022. Face-to-face semi-structured interviews were conducted,and the Colaizzi 7-step analysis method was used to summarize and refine the themes. Results 4 core themes were identified:parents of NSSI adolescents experienced various negative emotions in caregiving,including helplessness,confusion,panic,self-blame,anger,and grief;parents created a positive family environment through acquiring relevant knowledge,improving communication,and increasing companion time;some parents lacked effective coping strategies and tended to use violent communication,strict prohibitions,or indulgence as a single strategy;external support was insufficient,and parents hoped to receive more help from schools,hospitals,and communities. Conclusion Parents of NSSI adolescents experienced complex negative emotions,and attempted to create a positive family environment to reduce their adolescent’s NSSI behavior. However,they still lacked effective coping strategies and hoped to receive more external support. Healthcare professionals should provide emotional support and positive guidance to parents,explore effective family intervention strategies,and collaborate with families,schools,and communities to promote the physical and mental health of adolescents.

Objective To investigate the effect of Xiaoyu Tongluo fumigation moxibustion on the prevention of lower extremity DVT in patients with qi stagnation and blood stasis syndrome after major orthopedic surgery. Methods On the basis of literature review and expert meeting discussion,the intervention plan was constructed. A total of 132 patients with qi stagnation and blood stasis syndrome after major orthopedic surgery who were hospitalized in the joint department of an orthopedic hospital in Zhejiang Province from November 2021 to November 2022 were selected as the research subjects by stratified sampling method. They were divided into a Xiaoyu Tongluo fumigation moxibustion group,a moxibustion group and a control group,with 44 cases in each group. On the basis of the control group,the moxibustion group was treated with Xiaoyu Tongluo fumigation moxibustion;the moxibustion group was treated with moxibustion on the basis of the interventions in the control group,and the control group was given routine nursing measures to prevent the formation of DVT. The incidence of lower extremity DVT before discharge,coagulation function indexes on the 1st and 7th day after operation and daily pain score were compared among the 3 groups,and the intervention effect was evaluated. At the same time,the safety and feasibility of the intervention program were evaluated by the occurrence and completion rate of adverse reactions. Results There was no lower extremity DVT in the Xiaoyu Tongluo fumigation moxibustion group. The incidence of lower extremity DVT was 4.65 % in the moxibustion group and 16.67 % in the control group. The difference in the incidence of lower extremity DVT between the Xiaoyu Tongluo fumigation moxibustion group and the control group was statistically significant(P<0.05). Before and after the intervention,there were statistically significant differences in D-dimer,activated partial thromboplastin time,fibrinogen and prothrombin time among the 3 groups(P<0.05). After the intervention,there were significant differences in D-dimer,activated partial thromboplastin time,fibrinogen and prothrombin time among the 3 groups(F=41.885,P<0.001;F=247.377,P<0.001;F=32.626,P<0.001;F=17.056,P<0.001). The pain scores of the 3 groups were statistically significant on the 6th and 7th day after operation(P<0.05). There was no adverse reaction during the intervention period in the moxibustion group and the moxibustion group,and the completion rate was 97.73%. Conclusion Compared with moxibustion, Xiaoyu Tongluo fumigating moxibustion can more effectively reduce the incidence of DVT in lower limbs of patients with qi stagnation and blood stasis syndrome after major orthopedic surgery,effectively improve the hypercoagulable state of patients,and continuously relieve the pain of affected limbs. The intervention program has good safety and feasibility.

Objective To investigate the current situation of cancer-related cognitive impairment(CRCI) in patients with ovarian cancer,analyze its influencing factors,and explore the correlation between subjective and objective assessment of cognitive impairment. Methods From May 2021 to February 2022,155 patients with ovarian cancer from Shanghai Cancer Center were investigated by general information questionnaire,the Montreal Cognitive Assessment Scale(MoCA) and the Perceived Deficit Questionnaire(PDQ). Results Ovarian cancer patients had a high incidence of CRCI. The average score of MoCA was (24.14±4.02),and the incidence of CRCI was 65.8%. The decrease in abstraction,delayed recall,and visuospatial/executive functions were the most signi-ficant. The average score of PDQ was 17.35±10.67,with retrospective memory and planning/organization dimensions being the most severe. Binary logistics regression analysis showed that education and recurrence were influencing factors of MoCA(P<0.05). Multiple linear regression analysis showed that education,living status and taking oral medication significantly affected the score of PDQ. The score of PDQ were significantly negatively correlated with the score of MoCA(r=-0.210,P=0.009). Conclusion The prevalence of CRCI is high in patients with ovarian cancer,mainly in the memory,abstraction,and visual space and executive function. It is recommended that healthcare workers could incorporate CRCI into the symptom management of patients with ovarian cancer,and implement evidence-based interventions to those with high-risk factors to improve their quality of life.

Objective To retrieve and obtain the relevant evidence of swallowing function training for patients with head and neck cancer undergoing radiotherapy,and to summarize the best evidence for clinical references. Methods We systematically searched National Guideline Clearinghouse(NGC),Guidelines International Network(GIN),Scottish Intercollegiate Guidelines Network(SIGN),Joanna Briggs Institute(JBI),Cochrane Library,PubMed,UP To Date,Web of Science,Embase,CNKI,Wanfang Database,medlive for all evidence of swallowing function training in head and neck cancer patients undergoing radiotherapy,including clinical recommended practice,guidelines,expert consensuses,systematic reviews and randomized controlled trials.The retrieval period was from the inception of databases to August 2022. Results A total of 20 articles,including 1 clinical recommended practice,2 guidelines,5 expert consensuses,7 systematic reviews,and 5 randomized controlled trials. Through the evidence extraction and integration of the included literature,27 pieces of best evidence were finally summarized from the following 8 aspects,namely training purpose,training subject,training time,preparation before training,training scheme,radiotherapy suggestions,quality control and effect evaluation. Conclusion 27 pieces of best evidence were obtained based on the evidence summary method. In the process of nursing practice,nurses should select evidence in combination with specific clinical situations and patient needs,and relevant evidence should be updated in a timely manner.

Effects of non-nutritive breast sucking on the oral feeding ability of very low birth weight preterm infants

Objective To investigate the effect of non-nutritive breast sucking on oral feeding ability in very low birth weight preterm infants. Methods From July 2020 to June 2021,148 very low birth weight preterm infants admitted to the NICU of a tertiary children’s hospital in Shanghai were consecutively included,and they were divided into an experimental group and a control group with 74 cases in each group by block randomization. After the mechanical ventilation weaned,infants in the experimental group received non-nutritive breast sucking once a day for 5 minutes each time,and sucked the pacifier for 5 minutes before each tube feeding. The infants in the control group only sucked the pacifier for 5 minutes before each tube feeding. The time to achieve full oral feeding,the corrected gestational age at full oral feeding,the score of the Breastfeeding Behaviour Scale for premature infants,the days of breast feeding,and the breast feeding rate on the day of discharge were compared between 2 groups. Results There were 2 cases dropped in the experimental group and no cases dropped in the control group. The time to achieve full oral feeding was(12.29±4.12) d in the experimental group and(16.87±7.42) d in the control group. The difference between 2 groups was statistically significant(P<0.001). The corrected gestational age at the time of full oral feeding was(36.45±1.54) weeks in the experimental group and(36.13±4.59) weeks in the control group,and there was no significant difference between 2 groups(P=0.574). At the time of discharge,the Breastfeeding Behavior Scale score was[12.0(10.0,13.0)] in the experimental group and[6.0(1.5,8.5)] in the control group. The difference between 2 groups was statistically significant(P<0.001). During hospitalization,the days of breastfeeding in the experimental group were[47.0(33.8,62.8)] d,while those in the control group were[37.5(19.0,56.3)] d. The difference between 2 groups was statistically significant(P=0.021). The breastfeeding rate on the day of discharge was 84.7% in the experimental group and 66.2% in the control group,and the difference between 2 groups was statistically significant(P=0.012). Conclusion The implementation of non-nutritive breast sucking can shorten the time to achieve full oral feeding in very low birth weight preterm infants,promote their breastfeeding behaviour,increase the days of breast feeding with their own mother’s milk during hospital stay,and improve breast feeding rate.

To summarize the nursing experience of a patient with recurrent upper gastrointestinal bleeding combined with acute antibody-mediated rejection after kidney transplantation. Nursing points:early identification of upper gastrointestinal bleeding,active prevention of hypovolemic shock;close observation of complications after blood transfusion;prevention of deep vein thrombosis and internal jugular vein thrombosis;providing staged nutritional support to maintain the stability of internal environment;early identification of acute antibody-mediated rejection and initiation of emergency plan;management of plasma exchange;precision nursing of immunosuppressive drugs;the whole process of psychological counseling was given to relieve the negative emotions of patients. After 81 days of active treatment and intensive care,the patient recovered and was discharged from the hospital.

Objective The purpose is to translate the Hematology Information Needs Questionnaire(HINQ) into Chinese,and to test its reliability and validity. Methods The cross-cultural adaption guide of Beaton was used to translate the scale. The reliability and validity of the HINQ were tested through a survey of 362 patients with hematologic neoplasms in a tertiary specialized hospital in Tianjin by convenience sampling from October to December 2021. Results 345 valid questionnaires were collected. The Chinese version of the scale included 7 dimensions of disease(with 10 items),treatment(with 19 items),etiology(with 3 items),sleep and physical changes(with 6 items),self-care(with 7 items),medical tests and prognosis(with 11 items),psycho-social aspect(with 6 items),with a total of 62 items. In exploratory factor analysis,the cumulative variance contribution of 7 common factors was 73.963%. The item-level content validity index of the Chinese HINQ is 0.830~1.000,the scale-level content validity index is 0.949;Cronbach’s α coefficient for the overall scale was 0.986,and the Cronbach’s α coefficients of each dimension ranged from 0.903~0.976; the test-retest reliability of the total scale was 0.896,and the split half reliability coefficient was 0.917. Conclusion The scale has good reliability and validity,and it can be used as an assessment tool for the information needs of patients with hematologic neoplasms.

Objective To retrieve,evaluate and summarize the evidence related to blood collection via central venous access devices for clinical references. Methods Systematic search was conducted in related websites and databases. The search period was from the establishment of the databases to December 7,2021,and the clinical decisions,systematic reviews,guidelines,evidence summaries,and expert consensuses were included. The quality of the literature was independently evaluated by evidence-based trained investigators and combined with professional judgment to extract information from the literature that met the quality standards. Results A total of 7 articles were included,including 6 guidelines and 1 systematic review. The best evidence included a total of 27 pieces of evidence in 4 areas,namely training of health care personnel,blood collection methods,collection of special samples,and quality control strategies for blood collection. Conclusion In the process of evidence transformation,medical staff should comprehensively consider the clinical contexts and individual factors of patients,and choose the best evidence,optimize the clinical behavior of blood specimen collection through central venous access devices,so as to improve the standardization and safety of blood specimen collection.

Objective To construct a nursing plan for the quality of bowel preparation of diabetic patients,and to explore effects of its application. Methods The Delphi expert consultation method was used to construct a nursing plan for the intestinal preparation of diabetic patients. The plan includes 4 times of intestinal preparation intervention,5 key points of intestinal preparation intervention and 15 guiding contents of nursing intervention. 300 patients who underwent colonoscopy in the digestive endoscopy center of a tertiary general hospital in Fujian Province from August 2020 to January 2021 were selected as the research subjects,using the experimental research method. The research participants are grouped in chronological order,and finally 150 cases were included in the actual research subject control group and 150 cases in the experimental group. The control group was given regular bowel preparation guidance,and the experimental group used diabetic colonoscopy on the basis of routine care. The adequacy rate of bowel preparation,the detection rate of adenoma,the compliance of bowel preparation and the occurrence of adverse reactions of the 2 groups were compared. Results The bowel preparation adequacy rate,colon adenoma detection rate,and bowel preparation compliance ratio of the experimental group were all higher than those of the control group(P<0.05). The number of cases of nausea,abdominal distension and hypoglycemia in bowel preparation in the experimental group were Less than it in the control group(P<0.05). Conclusion The application of the intestinal preparation nursing intervention program for diabetic patients can effectively improve the bowel preparation adequacy rate,the detection rate of polyps,and the compliance of intestinal preparation;reduce the occurrence of adverse reactions during the intestinal preparation process,and alleviate the intestinal preparation of patients.

Objective The aim of this study was to perform a meta-synthesis of findings of myocardial infarction(MI)patients’ experiences of recovery after returning to society. Methods We searched databases including the Cochrane Library,Joanna Briggs Institute Library,PubMed,Embase,Web of Science,PsycINFO,CINAHL,CBM,WanFang,CNKI and VIP. All of the qualitative research on the real experience of MI patients returning to society after being discharged from the hospital were collected. The search time limit is from the establishment of the databases to November 2021. The JBI critical appraisal tool for qualitative research in Australia was used to evaluate the quality of the included studies,and the results were summarized and integrated by integrating methods. Results A total of 13 original studies were included. 47 findings were collected,and then the similar results were grouped into 11 new categories,and finally these categories were integrated into 3 synthesis results. Integration result 1:living with psychological loads,and the patients were facing physical and mental challenges. Integration result 2:reflecting on the value of life and strive to change one selves. Integration result 3:recognition of the world,and desire for outside help. Conclusion Although MI patients have been treated to save lives,they are still living under different degrees of pressure after discharging from the hospital. The future life is full of uncertainty,and each person’s coping ability is different. Health care workers and members of the public society need to pay more attention to their feelings and needs after returning to society,and to provide necessary health care knowledge and living skills to help them learn to coexist with the disease and better adapt to social lives.

Objective To understand the motivation of blood glucose management behavior changes in pregnant women with gestational diabetes mellitus(GDM),in order to provide suggestions and evidence for promoting good blood glucose management in GDM patients. Methods Purposive sampling method was used to select 28 pregnant women with gestational diabetes mellitus from September to December 2020 in 4 hospitals in Shanghai for semi-structured in-depth interviews. Content analysis was used to analyze and refine the theme. Results The motivation of blood glucose management behavior change of GDM pregnant women includes perceiving the severity of the disease,perceiving the risk of bad blood glucose management behaviors,and perceiving the benefits of healthy blood glucose management behavior. Conclusion Clinical nursing staffs can improve the awareness of GDM pregnant women on the severity of disease and the risk of bad blood glucose management behaviors,and improve the awareness of GDM pregnant women on healthy blood glucose management behaviors,so as to improve their motivation to change their poor blood glucose management behavior and unhealthy lifestyles.

The improvement effect of 11 kinds of exercise protocols on elderly patients with sarcopenia:a network Meta analysis

Objective To evaluate the effect of 11 kinds of exercise training on the improvement of physical function,muscle strength,and muscle mass in elderly patients with sarcopenia by network Meta-analysis. Methods A systematic search of Web of Science,PubMed,Embase,Cochrane Library,CINAHL,CNKI,Chinese Biomedical Literature Database(CBM),WanFang and VIP was carried out to identify randomized control trials about the effect of exercise training on the improvement of elderly patients with sarcopenia from inception to January 2022. The network meta-analysis was performed by Stata 15.0 software. Results A total of 34 pieces of research involving 2 199 patients and 11 exercise training were included. The results of network meta-analysis indicated that resistance training,Baduanjin,whole body vibration training,group comprehensive training,and home comprehensive training could improve the physical function of elderly patients with sarcopenia compared with non-exercise controls(P<0.05);resistance training and kettlebell training could significantly improve the grip strength of elderly patients with sarcopenia (P<0.05);resistance training could improve the skeletal muscle index of elderly patients with sarcopenia (P<0.05). Conclusion This study showed that resistance training is currently the best exercise intervention in elderly patients with sarcopenia,followed by group comprehensive training and whole body vibration training. However,more studies are still needed to further demonstrate.
